Case Management vs. Care Management

The role of the Case Manager is broader than health care or Care Management. A Case Manager addresses other social determinants of health and assists patients with areas like housing, legal support, protection from abuse, social needs etc. Care Management focuses on individualized level of care and your medical health care.
